What it does.

Ashwagandha is the adaptogen with the cleanest data. KSM-66 is the standardized extract that has been used in most of the trials worth reading. We dose at 600mg, which is the upper end of the range associated with measurable cortisol reduction and improvements in subjective stress and sleep quality.

I prescribe it for patients who are wired-tired — cortisol-dysregulated, sleep that is technically long enough but not restorative, anxiety that has the texture of a stress response rather than a mood disorder.

It is not a sedative. It does not work in an afternoon. Give it three to four weeks to see what it does.

Of the dozen botanicals labeled adaptogen, ashwagandha is the one with replicated trials in stress, sleep, and even some markers of testosterone in men. Most others have a single study and a press release.
